I'm guessing here, but it looks like he had some loose powder that caught fire. Like he'd just been working on a custom load and got so excited that he didn't clear the area.

The more I look at that the more I think that's what happened.

Note the smoke after he shoots - he's using black powder. He probably had a bunch of loads pre-measured but failed to protect them. Black powder burns slowly; he probably had a spark that blew out of the barrel late and touched off that secondary fire.

If he'd had any balls stacked in there it's a good bet he'll get an honorable mention for a Darwin Award.
