Depends. Usually however if the game is clearly lost. I will restart a game rather than spend another couple of hours playing a lost cause. I have been known to restart CIV V multiple times if my starting position is horrible.
If I'm multiplayer gaming, I do not quit the game unless my opponent would rather just wrap it up.

I have been known, when playing Crusader Kings 2, to reload when the rampant assassinations kill my entire lineage.
I only reload if my issue was caused by a problem with my inability to interface with the UI. I'll occasionally have an issue where I click the wrong button or highlight the wrong thing and that'll cause some horrible, horrible result in game. Then I'll reload.
(I'll also reload Total War games if one of my awesome high level assassin/spy/ninja type guys gets himself killed while burning down a gate or something. I just can't help myself.)
I have restarted Civ before when my starting position is horrible like others have said before. I might restart a game if it is no longer fun as I can see the writing on the wall for my loss.

I suggest just raising the family standings bonus up from it's current rating of
+jack'nsheeit to about +30. Then, perhaps, your whole family won't be trying to murder you because you're lazy or arbitrary or basically have any personality at all. :P
I sometimes use reload from a save point if I want to "test" a new game feature, etc. For example, I was playing a game of Mount and Blade (vanilla install) and decided (for the first time) to join one of the factions. I chose the Keshik because they were the weakest. I saved before I joined so that if it turned out that I didn't like either playing in a faction or playing that particular faction, I could always go back.
As also stated, I've restarted a campaign if I don't know the UI or game mechanics (Birth of America, and Patrician III).
And yeah...sometimes you just gotta reload in Civ.
Sadly, yes, I reload.
Skyrim and King Arthur come to mind as my biggest reload games. If I didn't reload Skyrim I probably wouldn't get past level 3.
King Arthur...man that game is brutal. I try to roll with the punches as much as possible. Most reloads are when I want to quick battle, but if the math gods are dogging me in quick battle then I'll reload and just fight the damn battle manually.
