Am itching for some realistic WWII online aerial combat...without the monthly subscription fees. Are there any active servers / groups / still playing IL-2 1946 multi-player?

Fire up hyperlobby. You should be able to find some activer servers in there.
I used to fly on Spits vs 109s and WarClouds quite a lot. They aren't nearly as active now due to the fracturing of the community with patch 4.11, 4.12, and CLOD. You should still be able to find people online during peak hours though.
During my days, I haven't found anything realisitc in the IL-2 1946 MP games I've joined (active and populated servers).
All of them are a mish-mash of palnes from different countries... like World of Tanks in the skies.
There's still a fair few players. As has been suggested, Hyperlobby is your best bet, but Xfire does a similar thing. Also you could look through the squad lists at http://www.mission4today.com/index.php?name=ForumsPro&file=viewtopic&t=4233 (http://www.mission4today.com/index.php?name=ForumsPro&file=viewtopic&t=4233) to see if any take your fancy.
